Health Care Plans
Each Medicaid card will have a Health plan and a customer service number.

Each Health plan has a limited number of transports each month for each client. You will need to check with your HMO for this information.

Appointments must be kept as "no shows" count as a visit. Continual "no shows" may result in your HMO dropping your family from transport privileges.

Note:To change your current health plan, open enrollment is in May and October. Those changing their enrollment in these months will not have to wait the mandatory 45 days for the change to become effective.
