How to Enroll in the Medicaid
HMO
By Mail:
When accepted to Medicaid, Michigan Enrolls will send all new
eligible clients an enrollment package to permit enrollment by mail.
By Telephone:
Clients can access the Call
Center to enroll toll free over the telephone. Michigan Enrolls - 888.367.6557
In Person:
Clients can come to a field office to meet the field counselors
who will assist in enrollment.

By Meetings:
Clients can attend informational meetings scheduled in community
locations to learn about managed care and the enrollment process, and
to complete the enrollment form.
By Home Visits:
Clients can request a home visit with field counselors to learn about
the enrollment process and complete the enrollment form. Health
Care Plans
Each Medicaid card will have a Health plan and a customer service
number. Each
Health plan has a limited number of transports each month for each
client. You will need to check with your HMO for this information. Appointments
must be kept as "no shows" count as a visit. Continual
"no shows" may result in your HMO dropping your family from
transport privileges. Note:To
change your current health plan, open enrollment is in May and
October. Those changing their enrollment in these months will not have
to wait the mandatory 45 days for the change to become effective.

Dental Visits:
HMO's may or may not provide transport for dental visits. Clients
need to call their health plans Customer Service number to check this
ever changing status. Vision
Care:
Referrals are made by each health plan to an optometrist that
likewise accepts the client's health plan. Those clients with
Community Choice need not see their physician first for a referral,
but may visit an optometrist who accepts Community Choice.
